A 22-year randomized controlled trial (RCT) conducted by you Weicheng, Dean of the school of Clinical Oncology of Peking University, evaluated the effects of Helicobacter pylori treatment, vitamin supplement and garlic supplement on the prevention of gastric cancer. The results showed that the continuous intake of garlic supplement for 7 years was related to the reduction of the risk of death from gastric cancer, which was similar to the two-week treatment of Helicobacter pylori and the intake of vitamin supplement for 7 years, But it has nothing to do with the decrease of incidence rate of gastric cancer. Linqu County, Shandong Province is one of the regions with the highest mortality rate of gastric cancer in the world. From 1980 to 1982, the mortality rate of gastric cancer was 55 / 100000 men and 19 / 100000 women (after age adjustment). In 1995, researchers recruited volunteers in Linqu County. After inclusion criteria screening, a total of 3365 volunteers from areas with high incidence of gastric cancer were finally included as the subjects of RCT.

The age of 3365 subjects ranged from 35 to 64 years. Among them, 2258 subjects with seropositive Helicobacter pylori antibody were randomly assigned to three intervention measures or placebo group, which were Helicobacter pylori treatment group, vitamin supplement group and garlic supplement group; 1107 H. pylori negative subjects were also randomly assigned to vitamin supplement group, garlic supplement group or placebo group.

After 22.3 years of follow-up from 1995 to 2017, 151 gastric cancer patients and 94 gastric cancer deaths occurred in all 3365 subjects. Among them, 79% of gastric cancer patients and 81% of gastric cancer deaths came from people who were positive for Helicobacter pylori at baseline.

Helicobacter pylori treatment and vitamin supplements can significantly reduce the risk of gastric cancer. After multivariable adjustment, the risk of gastric cancer was 52% (P < 0.001) lower in participants treated with H. pylori than in the control group, and 36% (P = 0.02) lower in participants taking vitamin supplements than in the control group. In terms of the proportion of gastric cancer, participants taking garlic supplements were slightly less than those in the control group, but the difference was not significant.

From the perspective of gastric cancer mortality, the three intervention measures are their protective factors, which are statistically significant. Helicobacter pylori treatment reduced gastric cancer mortality by 38%, vitamin supplement intake by 52% and garlic supplement intake by 34%. However, no association was found between the above three interventions and deaths from other cancers or cardiovascular diseases.

Further Kaplan Meier curves showed that the effect of Helicobacter pylori treatment and vitamin supplement intake on gastric cancer mortality became more obvious after about 8 years, while garlic supplement intake became more obvious after about 12 years.

However, the researchers also said that comparing the therapeutic effects of the three intervention methods in reducing the risk of gastric cancer incidence and death, it was found that there was no significant heterogeneity among the three methods. In other words, the effect of garlic supplements may be slow, but the effect will be gradually obvious and can last for 22 years!

In conclusion, this study found that 2 weeks of Helicobacter pylori treatment and continuous intake of vitamin supplements for 7 years can significantly reduce the risk of gastric cancer incidence and death, and the protective effect exists for a long time. Garlic supplementation did not significantly reduce the risk of gastric cancer, but it could significantly reduce the risk of gastric cancer death.

The results showed that among people who never drank alcohol, the protective effect of garlic supplement was more significant, and the risk of death from gastric cancer decreased by 67% compared with the control group!

At the end of the paper, the author of this study said that at present, Helicobacter pylori treatment is a more effective method to prevent gastric cancer, but it can not completely eliminate the occurrence of gastric cancer, and may also lead to drug resistance. Vitamin and garlic supplements have potential value in reducing the risk of morbidity and mortality of gastric cancer. Although the effect is slow and may take several years to have an impact, it is also a low-cost, safe and effective measure in the long run.
